摘    要:目的 探讨Bcl 2蛋白与P 糖蛋白在胃癌发生、发展和耐药性产生中的作用以及二者间的相互关系。方法 应用免疫组化SP法检测胃癌组织中的Bcl 2蛋白与P 糖蛋白表达情况。结果 胃癌中Bcl 2蛋白阳性表达率为 37.5%,高分化腺癌和组织学Ⅰ级阳性表达率显著高于未分化癌和组织学Ⅲ级 (P <0 .0 5) ;P 糖蛋白阳性表达率为 30 .0 %,淋巴结转移和癌细胞浸润浆膜及浆膜外者表达率显著高于无淋巴结转移和浆膜内浸润者 (P <0 .0 5)。胃癌中Bcl 2蛋白与P 糖蛋白共同表达率为 2 1 .4%(P <0 .0 5)。结论 Bcl 2参与了胃癌的发生 ;P 糖蛋白阳性表达提示胃癌细胞对化疗药存在原发性耐药 ,同时与癌细胞浸润深度、淋巴结转移有关。胃癌中Bcl 2蛋白、P 糖蛋白的表达可能有一定的联系

Study on the relationship between expressions of Bcl-2 protein and P-glycoprotein in gastric carcinoma
Abstract:Objective To evaluate the effects of Bcl-2 protein and P-glycoprotein(P-gp) in gastric carcinogenesis,tumor progression and drug resistant occurrence of gastric carcinoma. The relationship between Bcl-2 protein and P-gp was explored. Methods The expressions of Bcl-2 protein and P-gp were examined with SP immunohistochemical technique in 70 cases of gastric carcinoma. Results Bcl-2 protein expression rates were as the followings:35.7 % (25/70) in gastric carcinoma and 5% (1/20) in normal gastric mucosa. P-gp positive rates in tissues mentioned above were 30. 0 % (21/70) and 15 % (3/20), respectively. Bcl-2 positive rate in well differentiated adenocarcinomas were significantly higher than those undifferentiated carcinomas(P < 0.05). Bcl-2 positive rate in carcinomas whose histological grade was I were significantly higher than those in grade m (P < 0.05). In patients with lymphonode metastases, P-gp positive rate were significantly higher than those in patients without lymphonode metastases(P < 0.05). The P-gp positive rates in the caranomas with serious coat involvement were significantly higher than those in carcinomas without serious coat involvement(P <0.05). The co-expression rate of bcl-2 protein and P-gp in gastric carcinomawere 21.4% (15/70)(P<0.05). Conclusion Bcl-2 protein play a role in gastric carcinogenesis. P gp expression indicates there is primary drug resistance in gastric carcinoma and it is belived to becorrelated with poor prognosis. It may has the effect to motivate the diffussional transference andmetastasis of gastric carcinoma. There was significant relation between Bcl-2 protein and P-gp expression
