原发性高血压病血栓素A2-前列环素平衡及血小板功能改变的探讨

本文对63例正常人,126例原发性高血压(EH)患者的血浆TXB2、6-酮-PGF_(1a)及血小板聚集率进行了测定,并对单纯性高血压及合并心脏损害或缺血性脑血管病患者作了分析。结果表明,EH血浆TXB2水平较正常组明显升高。6-酮-PGF_(1a)水平则低于正常组,血小板聚集性较正常组高。单纯高血压组TXB2、血小板聚集率均低于有合并症组。提示EH存在TXA2-PGI2失衡及血小板功能亢进,其可能参与高血压的发生、发展,并为高血压合并心脏损害及缺血性脑血管症的病理基础之一。

CHANGES OF PLATELET FUNCTION AND THE BALANCE BETWEEN THROMBOXANE A2 AND PROSTACYLIN IN ESSENTIAL HYPERTENSION

Platelet aggregation and plasma concentration ofthromboxane B2 (TXB2) and 6-keto-prostaglandinF1α (6-keto-PGF1α), the stable nonenzymaticmetablites of thromboxane A2 (TXA2) andprostacyclin (PGI2), were respectively assayed in 126 pa-tients with essential hypertension and 63 normotensivesubjects to investigate the pathophysiolgical role ofplatelet function, TXA2 and PGI2 in essentialhypertension. In addition, we compared plateletaggregtion and plasma concentration of TXB2 and6-keto-PGF1α in hypertensive subjects with heartdamage or ischeamic cerebrovascular diseases andthose without complications. There were significantly in-creased platelet aggregation and plasma TXB2 concen-tration and significantly decreased plasma6-keto-PGF1α concentration in patients with essen-tial hypertension as compared with normotensive sub-jects Significantly increased plasmaTXB2, TXB2/6-keto-PGF1α ratio and plateletaggregation were also observed in hypertensive subjectswith heart damage or ischeamic cerebravascular dis-cases compared with hypertensive subjects without com-plications. The results indicate that there are platelethyperactivity and imbalance between TXA2 and PGF2in patients with essentidl hypetrtension, which may con-tribute to the maintenance of blood pressure elevationand the development of heart and ischeamiccerebrovasular complications in patients with essentialhypertension.
