腹部手术围手术期应用乌司他丁的作用及其机制

目的　探讨围手术期应用乌司他丁减少手术对机体损伤的作用及其机制。方法　腹部手术患者 2 0例为实验组 ,2 0例为对照组 ,检测两组术前 1d ,术后 1、3、5d血清TNF α、IL 6、IL 8、BUN及血肌酐 ,并观察两组患者术后体温及白细胞计数恢复正常的时间。结果　研究组与对照组相比较 :①术后TNF α、IL 6和IL 8研究组明显低于对照组(P <0 .0 5 ) ;②术后血肌酐研究组明显低于对照组 (P <0 .0 5 ) ;③术后第 1天和第 3天体温研究组明显低于对照组 (P<0 .0 5 ) ;④白细胞计数术后第 3天研究组明显低于对照组 (P <0 .0 5 )。结论　围手术期应用乌司他丁可通过抑制炎症反应防御组织损伤。

The effect and mechanism of ulinastatin application in abdominal surgery during perioperative period

Objective To explore the effect a nd mechanism of ulinastatin application during perioperation in abdominal surger y. Methods Twenty patients administered with ulinastatin serve d as study group and twenty ones without ulinastatin treatment served as control group. Serum TNF-α, IL-6, IL-8, BUN and serum creatinine were detected in both groups before and after operation. The normalization time of body temper ature and leukocyte count were observed. All data analyses were performed using SPSS. Results ① Serum TNF-α, IL-6, and IL-8 concentratio ns in study group were significantly smaller than those in control group on the 1 st, 3 rd and 5 th day after operation (P<0.05); ② Serum cr eatinine level in study group was significantly lower than that in control after operation (P<0.05); ③ Body temperature of the patients of study group was significantly lower than that of control on the 1 st and 3 rd day afte r operation (P<0.05); ④ Leukocyte count of study group was significantly le ss than that of control on the 3 rd day after operation (P<0.05). Conclusion Administration of ulinastatin during perioperati ve period in abdominal surgery can protect patients from damage of tissues by su ppressing inflammatory reaction.
