高原高血压的动态血压变化

摘    要:目的研究高原高血压的动态血压特点与变化规律。方法采用国产MPG1型动态血压监测仪,对平原医学体检血压正常的进驻海拔4 800 m高原施工人群中36例男性高原高血压患者进行标准化动态血压检测。结果高原高血压以白昼血压和舒张压升高为主(舒张压均值在全天、白昼均达高血压标准而收缩压只在白昼达到标准,夜间二者均下降到正常范围),单纯舒张期高血压所占比例显著高于单纯收缩期高血压(P<0.01),其昼夜节律仍呈杓形变化曲线,但夜间下降百分率仍<10%,心率与收缩压、舒张压、平均压及心率血压乘积均呈正相关关系(P<0.01)等特点。结论高原高血压的动态变化节律主要受机体耗氧程度的影响,即与低氧性增压反应有关,其防治重点仍应放在减少氧耗、改善低氧这一核心问题上。

Abstract:Objective To research the characteristic and changing regulation of ambulatory blood pressure monitoring in high altitude hypertension.Methods The builders in plain entered into elevation 4 800 m area after they were examined normal by medicine,36 person of them who had high altitude hypertension adoped standard ambulatory blood pressure monitoring.Results The elevation of diurnal blood pressure and diastolic pressure is of great value in high altitude hypertension,the percentage of the diastole high altitude hypertension is higher than systole high altitude hypertension(P<0.01),its circadian rhythm is curve like ladle shape,but the rate of nocturnal blood pressure's decreasing is still less than 10%,the heart rate has positive related relation with the systolic pressure,diastolic pressure,mean arterial pressure and the heart rate-blood pressure product(all P<0.01).Conclusion The body consuming oxygen degree has influence on dynamic rhythm of the high altitude hypertension greatly,it is also to say that it is related to the hypoxia pressure reaction,its focal point of prevention and cure should still be put into decreasing oxygen consuming and amliorativing the hypoxic state.
