猪肝细胞-中空纤维生物人工肝治疗犬缺血性暴发肝衰的实验研究

摘    要:为了初步探索生物人工肝(Bioartificialliver,BAL)治疗暴发性肝衰(Fulmi-nanthepaticfailure,FHF)的方法与疗效,为临床应用提供依据,我们建立了猪肝细胞是液-中空纤维体外BAL,并对缺血性FHF犬进行了治疗。对照组7例,治疗组6例,在入肝血流完全阻断后,对照组用无肝细胞的中空纤维全血液灌流5h,治疗组用在中空纤维盒外间隙植入3.15×109~3.60×109个活猪肝细胞的BAL血液灌流5h。结果表明,两组犬的血ALT、AST、LDH均进行性增高,两组间无显著差异(P>0.05),但治疗组的血Ammonia、TBIL、BUN水平明显比对照组低,血Glucose、FIB、动脉收缩压明显比对照组高,PT明显比对照组短(P<0.01)。说明这种BAL可产生维持生今的多种肝功能,治疗FHF是有效的。

Abstract:To detect the way of treatment forFHF by BAL and its effect and provid the basis forclinical application,we made the BAL consisting ofporcine hepatocytes and hollow-fiber, and treated the ischemic FHF dogs with it. There were 7 dogs in thecontrol group and 6 in the treatment group. Afterhepatic devascularization the control group dogswere attached to the FB-130A hollow-fiber modulealone and hemoperfused for sh,the treatment groupdogs were attached to the BAL of FB-130A hollowfiber module innoculated with 3. 15 × 109~ 3. 60 ×109 porcine hepatocytes and hemoperfused for sh.The results indicated that blood levels of ALT,AST, LDH increased progressively and there wereno significant difference between the two groups(P > 0. 05). But blood levels of ammonia, TBIL,BUN in treatment group dogs were obviously lowerthan those in the control group dogs. Blood levels ofglucose,FIB and arterial systolic pressure in treatment group pore higher than those in the controlgroup. PT was shorter in the treatment group (Pado. 01). The time of presenting hepatic coma waslater and the living time was longer (18 1 3. sh vs11. 8±4. 3h, P <0. 01 )in the treatment group. Ourresults suggest that the BAL of hollow-fiber module innoculated with porcine hepatocytes could produce a lot of hepatic functions which would maintainthe needs of life and is effective in treating FHF.
