摘    要:目的 比较脑钠肽前体(pro BNP)水平、心功能分级(NYHA)、经超声心动仪测定的左室射血分数(UCG EF)、6 min步行试验(6 MWT)在老年心衰患者心功能评定中的应用价值。方法 测定不同基础心脏病的老年心衰患者治疗前后的pro BNP水平,并与常规临床评估方法NYHA、UCG EF、6 MWT进行比较。结果 随治疗后心衰客观指标的好转,pro BNP水平下降,pro BNP心功能状态分级也降低,与pro BNP心功能状态分级比较,UCG EF心功能状态分级与其接近,其次为NYHA,6 MWT差异最大。结论 pro BNP是评定老年心衰患者心功能并判断预后较为可靠的指标,尤其当合并肺部疾病采用其他方法评定有困难时更有价值,其次为UCG EF。

Abstract:Objective To determine the effect of pro-brain natriuretic peptide (pro-BNP) and other methods on cardiac function of aged patients with heart failure. Methods pro-brain natriuretic peptide level was tested before and after the treatment in the aged patients with heart failure, and was compared with other route methods such as New York Heart Associationa (NYHA), Ultracardiogram-Ejection Fraction(UCG-EF), the six minute walk test (6-MWT). Results Pro-BNP level changed with the degree of patient's symptoms, highest in the patients with severe heart failure and decreased after the treatment. Conclusion Pro-BNP level was a good marker to evaluate the cardiac function and prognosis of aged patients with heart failure, especially under the condition that route methods could not evaluate the cardiac function of aged patients with heart failure and pulmonary diseases.
