污染及感染环境影响小血管吻合的实验研究

摘    要:用金葡菌在两组大白鼠(共120只)腹股沟区造成污染或感染模型,在污染或感染环境下行股动、静脉显微吻合。观察吻合血管15d通畅情况。结果显示:在污染区经清创后,吻合小血管15d通畅率为动脉90.6%;静脉88.5%。感染区内小血管吻合15d通畅率明显下降;动脉29.1%;静脉43.3%。因而认为:在污染环境下尽早清创后,行小血管吻合对通畅率影响不大。在感染区内,则应避免同有炎症的小血管吻合。必要时,移植正常血管,跨过感染灶,在正常组织内行小血管吻合。

Abstract:120 white rats, divided into twogroups, were made into a contaminative or infectious model respectively by using staphylococcus aureus in grion area. Femoral artery and vein were mi-croanastomosed under the contaminative or infectious environment. The results showed that afterdebridement in contaminative area, the free circulation rate of artery and vein 15 day postoperativelywas 90.6% and 88. 5% respectively, while that ininfectious area was 29. 1 % and 43.3% respectivelywhich dwindle significantly compared with the former. Therefore, we considered that the anastomosisof small vessels was feasible in contaminative areaafter debridement as early as possible. In infectiousarea, the anastomosis with inflammation small vessel should be avioded, but it could be anastomosedby astriding the area with normal vessel transplantationn.
