原发性十二指肠恶性肿瘤手术治疗(附31例报告)

本文总结分析了我院57～86年,经手术及病理证实的原发性十二指肠恶性肿瘤31例,其中根治术17例,局部切除4例,内引流术10例,术后存活最长达11年之久。原发性十二指肠恶性肿瘤在临床上与壶腹周围癌,胰头癌不易区别。本文就其发病率,诊断方法及治疗效果进行分析讨论。强调早期诊断是提高术后五年生存率的关键,并指出,局部切除对年老、体弱、局限性病变的病人亦能提高其术后生存率。

OPERATIVE TREATMENT OF PRIMARY DUODENAL MALIGNANT TUMOR-31 CASES REPORT

This paper analyses and summarizes 31cases of primary duodenal malignant tumo-rs during 1957～1986,the diagnosis beingconfirmed by operative findings and patho-logic characteristics(radical operation in 17cases,local or palliative resection in 4 cases,internal drainage in 10 cases).The longestpostoperative survival reached 11 years.Itis difficult to differentiate the diagnosisof primary duodenal malignant tumor fromthe cancer of periampulla and the bead ofthe pancreas.The purpose of this paper isto analyse and discuss the incidence,diagn-ostic methods and the effects of the opera-tion on primary duodenal malignant tumor.It was emphasized that the early diagnosiswas the key procedure to prolong the posto-perative five-years survival.It was alsoproposed that local resection and palliativeOperation be taken for the old and weakenpatients or the patients with localized lesi-ons to prolong the survival time.
