青少年血清瘦素水平与肥胖、血糖及胰岛素的关系

目的观察青少年血清瘦素水平与肥胖程度、糖、胰岛素及脂代谢变化的关系。方法对194名学生进行了OGTT(75 g葡萄糖)及胰岛素释放实验,测定瘦素、血脂、空腹和餐后2 h血糖、胰岛素水平并计算Homa胰岛素抵抗指数。结果女性血空腹血清瘦素水平明显高于男性,且按体重分组后,无论在肥胖组和正常体重组,女性血清瘦素水平仍明显高于男性。肥胖组血中血清瘦素明显高于正常体重组。血清瘦素水平与BMI、FIns、PIns2h和Homa胰岛素抵抗指数呈正相关。而与FPG、PG2h无相关性。在正常体重组,血清瘦素水平尚与TG呈正相关。在男性血清瘦素水平还与TC、TG、LDL呈正相关。结论肥胖青少年血清瘦素水平升高,血清瘦素水平与血脂、血压之间有复杂的相互关系,血清瘦素水平与空腹、餐后胰岛素水平和胰岛素抵抗指数有相关关系,说明瘦素在肥胖相关的高胰岛素血症和胰岛素抵抗发生中可能起作用。

The relationship between juvenile serum leptin levels and obesity, serum glucose as well as insulin concentrations

Objective To investigae the relationship between plasma leptin levels and obesity,plasma glucose,lipid as well as insulin concentration.Methods 194 students aged from 12-18 were selected.The assessments included serum leptin and lipid concentration,a 75g OGTT and insulin releasing test.Blood leptin,glucose,insulin and the blood lipids were measured.Results As compared with the control group,serum leptin,Fins(fasting insulin),Pins2h(2-hour postprandial insulin) and insulin resistance index(IR) in the obesity group were highly elevated and statistically distinguished by the t-test(P<0.05).Serum leptin levels were positively correlated with Fins,Pins2h and IR.Conclusion Serum leptin levels in juvenile obesity is high. The correlation between hyperleptinemia and hyperinsulinemia suggests a possible role for leptin in obesity-related insulin resistance pathogenesis.
