正常人脑氢质子磁共振波谱的研究

目的建立正常人脑额叶、基底节和海马各部位氢质子波谱的正常值标准。方法对160例健康志愿者(青年组和老年组各80例)额叶、基底节和海马进行波谱研究,测定各部位NAA、Cr、Cho的峰下面积并计算NAA/Cr、NAA/Cho、NAA/(Cr+Cho)、Cr/NAA、Cr/Cho、Cr/(NAA+Cho)、Cho/NAA、Cho/Cr、Cho/(Cr+Cho)。结果正常人脑上述比值无性别和部位差异(P>0.05),青年组与老年组间部分比值有显著性差异(P<0.05)。结论氢质子波谱测定应以波峰下面积的比值作为诊断标准,不需要考虑部位和性别因素,但需考虑年龄因素。

Study of proton magnetic resonance spectroscopy in normal human brain

Objective To establish the normal standard value of the metabolites by studying ~1HMRS in frontal lobe,basal ganglia and hippocampus.Methods ~1H-MRS was carried out in right lateral of frontal lobe,basal ganglia and hippocampaus of 160 healthy subjects(80 young healthy subjects and 80 old healthy subjects).We measured the area within the spectra of N-acetyl-aspartate(NAA),creatine / phosphocreatine(Cr),choline compounds(Cho) in the right side of frontal lobe,basal ganglia and hippocampaus of the healthy subjects and studied NAA/Cr,NAA/Cho,NAA/(Cr+Cho),Cr/NAA,Cr/Cho,Cr/(NAA+Cho),Cho/NAA,Cho/Cr,Cho/(Cr+Cho).Results The ratios of NAA/Cr,NAA/Cho,NAA/(Cr+Cho),Cr/NAA,Cr/Cho,Cr/(NAA+Cho),Cho/NAA,Cho/Cr and Cho/(Cr+Cho) in the same parts of normal human brain showed no gender-related differences,and no significant difference was found in different parts of the same group(P>0.05);some ratios in the three parts between the young group and old group showed significant difference(P<0.05).Conclusion When using ~1H-MRS,the ratios of the area within the spectra of NAA,Cr and Cho should be taken as clinical diagnostic criteria.There is no need to consider the factors of gender and position when using the normal value,while the factor of age should be considered,however.
