不同类型右室双出口的外科治疗与对策

报告1996年4月至7月在上海新华医院学习期间，以及后来作者所完成不同类型右室双出口矫治本共9例，其中心内隧进修补使左室连接主动脉6例（包括右室流出适加宽4例，切口直接缝合1例，应用同种带辩动脉重建右室流出道1例）；右房内板障分隔或折流3例（其中上腔静脉横断后两端分别与右肺和肺总动脉即全腔肺动脉吻合2例，同种带瓣动脉连接右房与肺总动脉即改良Fontan术1例）。手术中无1例死亡。作者认为分型与畸形不同决定术式有利，保证左、右室流出道通畅是手术成功之关键，对伴发畸形的合理兼治是降低并发症和死亡发生率的又一重要环节。

SURGICAL DECISION IN DOUBLE OUTLET RIGHT VENTRICIE

The different kinds of double outlet right ventricle were reported in 9 patients. The types of operation: ①Intraventricular tunnel repeir in 6 cases, including right ventricle outflow widen-ing in 4, cut stitching in 1, right ventric1e outflow reconstruction with conduit valved homograft in 1.②Septation and deviation in right atrium baffle in 3,mcluding total cavopu1monary anastomosis in 2,modified Fontan procedure in 1. No death record-Based on other medical literature, the authors think that the types and deformation decide the way of op-eration, and keeping the ventricle outf1ow smooth is important, and reasonably treating the combined de-formation is crucial to decrease morbidity of compli-cations and mortality.
