摘    要:目的探讨Bcl-2、Bax蛋白在梗阻性黄疸大鼠肾小管的表达及其与肾小管上皮细胞凋亡的关系。方法60只SD大鼠,随机分为对照组(20只)和梗阻性黄疸组(40只)。结扎胆总管建立梗阻性黄疸模型,对照组仅剖腹游离胆总管。两组大鼠分别于术后第1、2、3、4周处死,检测血清尿素氮(BUN)和肌酐(Cr)的变化;观察肾脏病理组织学及超微结构改变;TUNEL法观察肾小管细胞凋亡;免疫组化Elivision二步法检测梗阻性黄疸大鼠肾组织中Bcl-2、Bax蛋白的表达。结果随着梗阻性黄疸时间的延长,血清BUN和Cr逐渐升高;肾小管凋亡细胞增多。梗阻性黄疸组与对照组的Bcl-2、Bax蛋白表达相比,有显著统计学差异(P<0.01)。经Spearman s相关性分析,肾小管细胞Bax蛋白表达与肾小管细胞凋亡呈正相关,而Bcl-2蛋白表达则与细胞凋亡呈负相关。结论梗阻性黄疸可以引起大鼠肾脏损害,肾小管细胞凋亡在梗阻性黄疸大鼠肾功能损害中发挥作用;Bcl-2、Bax蛋白对梗阻性黄疸大鼠肾小管上皮细胞凋亡有调节作用。

Abstract:Objective To investigate the relationship of the expression of Bcl-2 and Bax protein with apoptosis of renal tubular epithelia in rats with obstructive jaundice(OJ).Methods A total of 60 adult Sprague-Dawley(SD) rats were divided into two groups: OJ rats by ligating bile duct(OJ group,n= 40) and the sham-operation rats(the control group,n=20).The two groups of rats were sacrificed at postoperative 1,2,3 and 4 week,respectively.Then serum BUN and Cr levels were tested and renal histopathological and ultrastructural changes were observed.Apoptosis of renal tissues was assayed by TUNEL method.The immunohistochemical Elivision~(TM) technique was adopted for detecting the Bcl-2 and Bax protein expression.Results With the time of OJ prolonging,the value of serum BUN and Cr increased;apoptotic cells of renal tubular epithelia increased.There were obvious differences in Bcl-2 and Bax protein expression of between OJ and control groups(P<0.01).By the Spearman's correlation analysis,apoptosis of renal tubuli was positively related to Bax protein expression and negatively to Bcl-2 protein expression.Conclusion Obstructive jaundice can lead to renal injury;apoptosis has an important effect on renal function injury in rats with obstructive jaundice.Bcl-2 and Bax protein can regulate the apoptosis of renal tubular epithelia in rats with obstructive jaundice.
