血清、尿β_2微球蛋白测定对糖尿病肾脏病变的诊断价值

摘    要:本文用放射免疫法则定了49例糖尿病惠者血清、尿β_2m含量,并与正常人对照。结果表明,血清β_2m值在糖尿病伴蛋白尿组、其他并发症组显著升高,尿β_2m在糖尿病伴肾盂肾炎组、蛋白尿组和其他并发症组也明显增加。血清β_2m值与BUN呈高度正相关,与Ccr呈高度负相关,尿β_2m值与PSP的2小时排泄率呈高度负相关。而且在BUN和血Cr升高或尿蛋白阳性之前,即可出现β_2m的显著增加。提示血清、尿β_2m测定是检查糖尿病患者早期肾脏损害的一种灵敏方法。

Abstract:The concentrations of serum and urine beta 2 microglobulin (β_2m) were measured by radioimmunoassay in 49 cases of diabetes mellitus and compared with normal controls. The results showed that the level of serum β_2m in diabetics with proteinuria and other com- plleations was significantly increased (P<0.05 to 0.01) and the level of urine β_2m in diabetics with pyelonephritis, proteinuria and other complications was alsomarkedly raised (P<0.05 to 0.01). The serum β_2m showed a Positive correlation with BUN (r=0.6415, p<0.001), a negative correlation with Cr (r=-0.8962, P>0.001), and the urine β_2m showed a negative correlation with the PSP excretory rate in 2 hours (r=0.6928, P>0.001). The elevation of β_2m appeared before the abnormality of BUN, blood Cr and urine protein in some patients. It is considered that β_2m in serum and urine is a sensitive index of the early lesion of renal function in diabetics.
