B超评价急危重症患者的右心房压力

目的　评价二维超声检测急危重症患者的右心房压力的临床应用价值和可行性。方法　急诊收入ICU和 (或 )留观患者 32例 ,2 .5MHz超声心动图探头或 3.5MHz超声腹部探头 ,取剑下长轴 ,显示下腔静脉入右心房前 2cm内部位 ,分别测量吸气末和呼气末的下腔静脉血管内径 ,以此评估右心房压力。结果　缺血性心脏病伴左心功能不全 6例患者尽管伴有颈静脉充盈 ,却少有右心房压力增高。慢性阻塞性肺病 9例患者都有颈静脉充盈 ,但是约 1 3没有右心房压力增高。结论　用二维超声检测急危重症患者右心房内压力 ,适时、无创、易于操作 ,有临床应用价值和可行性

Assessment of right atrial pressure by echocardiography in emergency room and ICU

Objective To assess the clinical significance and feasibility of echocardiography to detect the right atrial pressure in patients presented to the emergency room and ICU. Methods Thirty two consecutive patients in emergency room and ICU underwent two dimensional echocardiography of right atrial pressure during 72 hours. End expiratory and end inspiratory IVC diameters and percent collapse were measured in subcostal long axis views within 2 cm of the right atrium.Results Six patients with ischemic heart diseases accompanied by left heart failure had slightly increased right atrial pressure, and about 30 percent patients with COPD did not have increased right atrial pressure. Conclusion Two dimensional echocardiography is a valuable and feasible means for evaluation of right atrial pressure in patients presented to the emergency room and ICU.
