Acute Appendicitis in the Aged—Report of 86 Cases

In 1961—1982, 86 aged patients with acute appendicitis were treated in our hospital, being 3% of the total patients with appendicitis during the same period of time. Of 86 patients, 20 were not operated on and 66 underwent surgery. The curative results were excellent in 73 cases, good in 9 and poor in 3. Death occured in one case. The results of analysis show that the important clinical feature of acute appendicitis in the aged is a high rate of perforation. In addition to the pathological changes of appendix, a late diagnosis and preoperative delay are responsible for the high perforation rate. For this reason, emphasis must be placed on an early diagnosis and operation to decrease the perforation incidence and postoperative complications. Appendicitis should be differentiated from cancer of the ileocecum and colon descendens in the aged.
