HtrA1在宫颈上皮内瘤样病变及宫颈鳞癌中的表达及临床意义

目的探讨HtrA1在宫颈鳞状上皮内瘤样病变(CIN)及宫颈鳞癌中的表达及临床意义。方法采用免疫组化S-P法对10例正常宫颈、28例CIN(其中轻度8例、中度10例、重度10例)、25例原位癌及40例宫颈浸润鳞癌标本中HtrA1的表达进行检测,并与临床病理分级等进行相关分析。结果正常宫颈组织中HtrA1无阳性表达。CIN、原位癌的阳性表达率分别为21.4%及60%,各组间差异有统计学意义(P<0.05)。浸润癌阳性表达率为52.5%,较原位癌组显著降低(P<0.05)。HtrA1阳性表达率与宫颈癌临床分期呈负相关(r=-0.622,P<0.05),与病理分级无关(P>0.05)。无淋巴结转移者HtrA1阳性表达明显高于有淋巴结转移者(P<0.05)。结论 HtrA1表达对宫颈癌的发生发展及评估预后有重要意义,可能是一种肿瘤抑制因子。

Expression of HtrA1 in cervical intraepithelial neoplasm and cervical cancer

Objective To study the expression of HtrA1 in cervical intraepithelial neoplasm(CIN) and cervical squamous carcinoma and its clinical significance.Methods In 10 cases of normal cervical tissues,28 cases of CIN(8 mild cases,10 moderate cases and 10 severe cases),25 cases of intraepithelial carcinoma of cervix uterus and 40 cases of invasive carcinoma of cervix uterus,the expression of HtrA1 was detected by immunohistochemical method S-P.Its correlation with clinico-pathological grade was analyzed.Results HtrA1 was negative in normal cervical epithelium.In CIN and intraepithelial carcinoma,the positive expressive rate of HtrA1 was 21.4% and 60%,with a significant difference(P<0.05).Expression of HtrA1 in invasive cervical carcinoma was 52.5%,significantly lower than that in intraepithelial carcinoma(P<0.05).The positive rate of HtrA1 had a negative correlation with clinical stage of the carcinoma(r=-0.622,P<0.05),but was not related to the histological grade(P>0.05).The positive rate of HtrA1 in the subjects without lymph node metastasis was much higher than that in those with lymph node metastasis(P<0.05).Conclusion HtrA1 is important in evaluating the development of neoplasm and prognosis of cervical carcinoma,and it may be an oppressor for the carcinoma.
