心阻抗图测定小儿心肌炎左室功能变化

本文采用心阻抗图测定了76例4岁～12岁心肌炎患儿的左室功能,并以54名同龄健康小儿作对照,以PEP、LVETI、PEP/LVET、Q-Z、SV、CO、CI、HI作为观察指标,有1项以上异常者共55人,占72.4%,各项指标中,PEP/LVET、PEP、Q-Z较LVETL、SV、CO、CI和HI敏感。若以PEP/LVET>0.35作为衡量心功能异常的主要指标,则慢性期心功能异常发生率最高(81.0%),其次为急性期(36.7%),而迁延期患儿PEP/LVET比值均在正常范围。重型患儿心功能均异常,且PEP/LVET>0.40,而心功能正常的患儿病情均较轻。这一检查方法在协助诊断、判断病情、估计预后及观察疗效等方面均有一定价值。

OBSERVATION ON THE CHANGES OF THE LEFT VENTRICULAR FUNCTION IN CHILDREN WITH MYOCARDITIS BY IMPEDENCE CARDIOGRAM

The left ventricular function of 76 children agedfrom 4 to 12 with myocarditis was examined byimpedance cardiogram and cotrolled with 54 cases ofcontemporary healthy children PEP, LVETIPEP/LVET ratio Q--z interval, SV CO, CI and HIwere used as the observation index Of the 76 caseswith myocarditis, there were 55 cases, each of themhad one abonormal index or more, accounting for 724% Among thesc observation indices PEP/LVET,PEP and Q--Z interval are more sensitive thanLVETI, SV, CO, CI and HI, If the ratio ofPEP/LVET more than 0.35 is considered as a main in-dex of judging abnormal left ventricular function, the fre-quency of abnormal cardiac fnction in chronic stage isthe highest (81. 0%), the second is the acute stage(36.7%), however, the ratio of PEP/LWET of all thepatients in prolonged stage was within normal rang Asto severe cases, their cardiac functions were all abnormal,and the ratio of PEP/LVET in each case was morethan 0.40. But the patients who had normal cardiacfunctions were all mild cases. It is suggested that thismethod be used for the assessment of diagnosis se-verity efficacy of therapy and prognosis of myocarditis.
