冠状动脉内支架置入术45例临床分析

摘    要:目的 分析 45例冠脉内支架置入术 ,探讨其临床应用价值。方法 男性 41例 ,女性 4例 ,平均年龄 55.8岁。共 48支靶血管内置入支架 49只 ,单纯型病变 5例 (1 0 .2 % ) ,复杂型病变 44例 (89.8% ) ,包括闭塞性病变 3例。 de novo置入 1 4例 ,suboptimal置入 1 4例 ,bail- out置入 1 7例。多支架置入 3例。结果 手术成功率 1 0 0 % ,均无严重急性血管并发症和出血并发症。冠脉造影 (CAG)随访 5例 ,平均随访 1 0 4 d,其中 1例出现再狭窄 (2 0 % ) ,再次行经皮穿刺冠脉腔内成形术 (PTCA)有效。结论 冠脉内支架植入术是一种安全有效的介入治疗技术 ,急性心梗者手术的成功标志着适应证范围进一步扩大

Clinical analysis of coronary-stent implantation(45cases)
Abstract:Objective To evaluate the results and benefits of stenting implanation in coronary artery disease patients.Methods From May 1995 to October 1998,49 introcoronary stents were implanted on 45 patients,including 14 cases with acute myocardial infarction.There were 41 male and 4 female.They ranged in age from 41 to 70 years(mean 55.8 years).There were 5 simpal lesions and 44 complex .Indications were:de novo in 14, suboptimal in 14, bail out in 17.Results The procedure success rate was 100% .No acute or subacute intro stent thrombosis and severe bleeding was observed in a total of 49 stents.5 cases were followed up by coronory angiography(CAG) for 104 d on the average.Restenosis was found in one case(20%).Percutaneous transluminal coronary angioplasty once again(PTCA) was effective for this one of stenting postprocedural restenosis.Conclusion Coronary stent is a safe and efficient interventional procedure.In additionwith the recent advances in technique,using high implantation pressure and combined of anticoagulant therapy,all these advances have resulted extended indications and reduced complications.But the long term benifit remains to be determined.
