新生儿缺氧缺血性脑病血浆与脑脊液肿瘤坏死因子水平变化的临床研究

为探讨肿瘤坏死因子 ( Tumor necrosis factor,TNF)与新生儿缺氧缺血性脑病的关系 ,应用放射免疫分析法测定 31例新生儿缺氧缺血性脑病 ( Hypoxic-ischemic encephalopathy,HIE)患儿和 1 2例正常新生儿血浆 TNF水平及 2 4例 HIE患儿脑脊液 TNF水平。结果表明 ,HIE患儿急性期血浆 TNF水平显著高于对照组 (P <0 .0 5 ) ,且随 HIE程度加重而增高。中、重度 HIE急性期与恢复期比较均具有显著差异 ( P <0 .0 1 )。 CT异常组血浆和脑脊液 TNF水平均高于 CT正常组 ,且血浆与脑脊液 TNF之间呈正相关关系 ,推论 TNF参与了 HIE的病理过程 ,测定 TNF可作为新生儿 HIE的监测指标。

A CLINICAL STUDY ON SERUM AND CSF LEVELS OF TUMOR NECROSIS FATOR IN NEOTNATAL HYPOXIC-ISCHEMIC ENCEPHALOPATHY

To study the serum and CSF levels of tumor necrosis factor (TNF) in neonatal hypoxic-ischemic encephalopathy (HIE),We measured serum levels of TNF in 31 HIE and 12 normal controls and CSF levels of TNF in 24 HIE by using radioimmunoassay.The serum levels of TNF in HIE at actute period were significantly higher than that in normal controls.and it was obviously related to severity of HIE.The serum levels of TNF in moderate and severe HIE was significantly higher than that at convalescence period.The serum and CSF levels of TNF in abnormal CT group were significantly higher than that in normal CT group.Rank correlationship show a significant relation-ship between serum levles of TNF and CSF levels of TNF. It suggests that TNF take part in pathologycal process of HIE and measurement of TNF should be considered as an index to monitoring HIE.The role of TNF in HIE should be explored.
