摘    要:目的研究术前一周开始肠内营养(enteral nutrition,EN)支持对胃癌患者术后营养状况、免疫功能及炎性反应的影响。方法将126例胃癌患者随机分为术前一周开始EN组(研究组)和术后早期EN组(对照组),分别按不同治疗方案进行营养支持至手术后第9天。所有患者均于术前、术后第1、10天测量体重、皮褶厚度、上臂围、血白细胞计数(WBC)、白蛋白(ALB)、前白蛋白(PA),C反应蛋白(C-reactive protein,CRP)、体液免疫(IgAI、gG)、T细胞亚群(CD4、CD8、CD4/CD8)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)等。结果术后第10天研究组前白蛋白和IgG水平高于对照组,IL-6低于对照组。结论对胃癌患者术前进行合适的EN支持,能更好地改善患者术后的营养状况、免疫功能并减轻患者机体炎性反应,更有利于促进患者恢复。
