胎儿下腔静脉直径与血流频谱的超声检测

摘    要:目的 研究胎儿下腔静脉直径和血流频谱的超声检测方法及意义。方法  75例孕妇 (孕周 2 2~ 4 1周 )行常规超声检查 ,对胎儿下腔静脉行二维及多普勒超声检查。结果 正常胎儿下腔静脉多普勒血流频谱图为三相型 :大“v”、小“e”波和反向“a”波 ;不同孕周胎儿下腔静脉的血流参数中 ,RI、PI、A/S、S/D无统计学差异 ,而TAM、Tmax、Tmin在妊娠早期 (<2 8周 )显著低于妊娠中晚期 (P <0 .0 5 ) ;下腔静脉直径随孕周增长而增大 (P <0 .0 1)。结论 多普勒超声探测胎儿下腔静脉血流频谱是完全可行的 ;测量下腔静脉直径及血流动力学参数时要考虑孕周数

Abstract:ObjectIve To study methods and significance of detecting diameter and flow spectrum of fetalinferior vena cava (IVC) by ultrasonography. Methods Seventy-five pregnant women of gestational age 22 - 41weeks were examined by common ultrasonography, then IVC was investigated by two dimensional and duplexDoppler ultrasonography. Results The Doppler flow spectrum of normal fetal IVC comprised three phases, big"v", small "e" and retrograde "a". During the flow parameters of fetal IVC of different gestational ages, therewere no statistical differences in RI, PI, A/S and S/D; whereas the values of TAM, Tmax and Train weresignificantly lower in early gestational age than in middle and late gestational age (P < 0.05). The diameter ofIVC increased with the progression of gestational age, with significant differences between the three groups (P<0.01 ). Oonclusion It is feasible to investigate the fetal IVC flow by duplex Doppler ultrasonography.Gestational age should be considered when the diameter and flow parameters of fetal IVC are detected by ultra-sonography.
