| 摘 要： | 目的探讨基质金属蛋白酶7(MMP-7)在肾细胞癌组织中的表达及其与临床病理因素及预后的关系。方法应用免疫组织化学SP法检测78例肾细胞癌组织及相对应的癌旁肾脏组织中MMP-7的表达情况。结果 MMP-7在癌旁肾脏组织中几乎不表达,在肾细胞癌组织中MMP-7阳性表达率为55.1%。MMP-7的异常表达在患者的不同性别、年龄、肿瘤大小和病理组织学类型及有无血尿、腰痛、肾盂浸润、肾静脉癌栓无显著性差异(P>0.05),与肾细胞癌包膜浸润、淋巴结转移有显著性差异(P<0.05),并与病理组织学分级、临床分期相关(P<0.05),有包膜浸润、淋巴结转移以及病理组织分级高、临床分期晚的肾细胞癌MMP-7表达率高于无包膜浸润、淋巴结转移和病理组织分级低、临床分期早的肾细胞癌(P<0.05)。MMP-7表达阳性病例术后生存时间显著短于阴性表达病例(P<0.05)。结论 MMP-7在肾细胞癌中有较高程度表达,并与肾细胞癌的浸润、转移及生存时间有一定的关系,检测MMP-7可作为判断肾细胞癌恶性程度及预后的指标。
