摘    要:目的 分析非小细胞肺癌癌性胸水细胞中影响免疫状态的 7种细胞因子的表达 ,初步探讨肿瘤局部免疫微环境对抗肿瘤免疫应答的影响 ,揭示肿瘤逃逸机制。方法 应用原位杂交技术检测非小细胞肺癌癌性胸水和结核性胸膜炎炎性胸水细胞中IL 2、INF γ、IL 1 2、IL 1 8、IL 1 0、IL 4、TGF β1mRNA表达 ,比较其差异并进行半定量分析。结果 非小细胞肺癌癌性胸水细胞中IL 1 0、TGF β1、IL 4表达者显著高于IL 2、INF γ、IL 1 2、IL 1 8,且显著高于对照组。结核患者表达的各细胞因子水平均较低 ,且各因子的表达细胞数量相互间无显著性差异。结论 非小细胞肺癌患者癌性胸水细胞中Ⅱ型细胞因子表达者占主导地位 ,IL 1 0和TGF β1共同高表达 ,提示二者可能共同作用 ,参与形成非小细胞肺癌患者免疫抑制状态

Abstract:Objective Analyzed mRNA expressions of 7 cytokines which influence the immune response in lymphocytes in pleural effusion of non-small cell lung cancer patients to evaluate the effect of local tumor microenvironment on anti-tumor immune response and explore the mechanism of tumor escape. Methods Detected the mRNA expression of IL-2, INF-γ, IL-12, IL-18, IL-10, IL-4 and TGF-β1 in lymphocytes in pleural effusion of non-small cell lung cancer patients and tuberculotic pleurisy patients on the single cell level by using in situ hybridization. Results In the pleural effusion of non-small cell lung cancer, the mRNA expression of IL-10, TGF-β1 and IL-4 were significantly higher than IL-2, IL-12, IL-18 and TNF-γ, as well as the control group. The cytokineexpression levels of tuberculotic pleurisy patients were very low, and there was no significant difference among them. Conclusion Type 2 cytokines were expressed predominantly in the pleural effusion of non-small cell lung cancer. The increased co-expression of IL-10 and TGF-β1 indicated that they might act jointly and play a critical role in the immunosuppression of non-small cell lung cancer.
