晚期直肠癌的姑息疗法

应用肠腔及痔上动脉化疗药物灌注后用TH胶堵塞痔上动脉,结扎双侧髂内动脉以阻断肿瘤组织的血供,治疗17例晚期直肠癌。经1年～7年随诊,对照观察表明:76.4%自觉症状好转,23.5%减轻,82.3%自我生活能力提高,88.2%粘液分泌及出血减少,原发灶生长趋于缓慢,病理显示细胞退形性变,癌细胞结构模糊,核仁不清,细胞萎缩,间隙增大,周围组织水样变性,生存期较单纯造瘘组延长2年左右。

THE PALLIATIVE TREATMENT OF THE RECTAL CANCER IN ADVANCED STAGE

Since 1983, We have treated 17 caces of the rectalcancer in advanced stage with chemotherapy byperfusing the intestinal cavity and the superior rectal ar-tery with anticarcinogen and then obstructing the superi-or hemorrhoid artery with TH glue, ligating the bilateralinternal iliac arteries in order to stop the blood supplyof the cancer. Through one to seven yers fllow-up serv-ice and control observation, 76.4% patients felt theirconscious symptoms take a turn for the better 23.5%patients felt their symptoms abate, 82.3% patients im-proved their ability to look after themselves. Themucus secretion and hamorrhage of 88.2% patients de-creased. The original focus tends to grow slowy. Pathol-ogy shows as follows: The cell of the cancer has retrograde affections. The cyto-architectionics is indistinct and thenucleolus is not clear. The cell atrophies and the cyto-intervals enlarge. The existent time will be about two years longerthan that of the simple fistulation.
