血清ACE2/Ang(1～7)与风湿性心脏瓣膜病患者心房颤动发生的相关性分析

目的分析风湿性心脏瓣膜病患者血清ACE2/Ang(1~7)的浓度与心房颤动发生之间的相关性,探讨ACE2/Ang(1~7)在风湿性心脏瓣膜病房颤发生中的作用,探寻房颤的预防及治疗的新靶点。方法收取风湿性心脏瓣膜病患者[46例,其中心房颤动(AF)组24例,窦性心律(SR)组22例]基本临床资料及外周静脉血,用ELISA法检测血清中ACE2、Ang(1~7)、AngⅡ的浓度,分析两组间的差异及相关性。结果 AF组可见以下改变:1左心房直径明显大于SR组[(60.70±3.08 vs.48.15±2.16)mm,P<0.05];2血清AngⅡ浓度[(45.88±2.87 vs.35.78±1.08)pg/mL,P<0.05]明显高于SR组,同时AngⅡ与左心房直径呈正相关(Pearson检验,P<0.05);3血清ACE2浓度[(7.87±0.74 vs.11.65±0.57)U/L,P<0.05]及Ang(1~7)浓度[(146.05±17.61 vs.321.71±36.50)pg/mL,P<0.05]显著低于SR组,与左心房直径呈负相关(Pearson检验,P<0.05);4血清Ang(1~7)与AngⅡ浓度呈负相关(Pearson检验,P<0.05)。结论在风湿性心脏瓣膜病患者中,ACE2/Ang(1~7)可能通过拮抗AngⅡ抑制心房的重构,对于房颤的发生具有保护作用。     

哇巴因对血管紧张素Ⅱ及其1型和2型受体mRNA在心肌中表达的影响

目的观察哇巴因(Ouabain)对血管紧张素Ⅱ(AngⅡ)及其1型(AT1)与2型(AT2)受体mRNA在心肌中表达的影响。方法60只SD大鼠随机分为3组,分别为Ouabain组[27.8 ng/(kg.d)Ouabain腹腔注射]、Losartan组[27.8 ng/(kg.d)Ouabain腹腔注射,同时给予Losartan 30 ng/(kg.d)灌胃]、对照组(生理盐水2 mL/d腹腔注射),每周测定鼠尾血压,共6周。应用放免法测定各组大鼠血浆及心肌中AngⅡ质量浓度,同时采用实时定量荧光PCR(FQ-PCR)观察心肌中AT1和AT2mRNA表达的变化。结果血浆中的AngⅡ质量浓度在Ouabain组及Losartan组均显著高于对照组(P<0.05),而Ouabain组心肌中AngⅡ质量浓度无明显变化。与对照组相比,Ouabain组及Losartan组心肌中AT1mRNA与AT2mRNA的表达明显上调(P<0.05)。结论外周长期、慢性给予Ouabain后可持续升高SD大鼠的血压,并使RAS系统激活,这一作用可能是通过AT1受体介导。给予Ouabain激活AT1受体的同时,AT2活性也增加。
